We appreciate your trusting BCVC to provide health care for your pets. We strive to provide quality customer service and excellent veterinary care for all your pets. For your visit, please call our office at 973-948-PETS (7387) to speak with a Client Care Specialist about scheduling an appointment.

BCVC now requires having both the NEW CLIENT INFORMATION FORM & FINANCIAL POLICY FORM as well as your pets' VACCINATION and MEDICAL HISTORY to our office PRIOR to scheduling a first visit appointment.

OPTIONS TO GET COMPLETED FORMS TO BCVC:

1.) Bring completed forms and pet records to our office during our business hour. Please call the office upon your arrival so a staff member can obtain the paperwork from you.

2.)Mail them to Branchville Country Vet Clinic, 12 Maple Ave., Branchville, N.J. 07826

3.) Email them to BRANCHVILLEVET@YAHOO.COM. To email forms, please make sure handwriting is legible, all sections are completed, and the scan or snapped picture is clear and shows the entire form or all pages of your pets' medical history.

Please complete and bring the UPDATING INFORMATION FORM for your visit.

FOR YOUR PETS

To ensure we address all your pets' needs, we require your pets' VACCINATION and MEDICAL RECORDS in our office

prior to BCVC scheduling their first visit.

Having previous medical history in advance allows us to determine the best care for your pet and can avoid duplication of services.

For your convenience, you may wish to use the PET RECORDS REQUEST FORM. Pet records may be obtained from previous veterinarians, breeders, adoption agencies, or in some cases, former pet owners.
